Yearbook Senior Ad - Full Page

To place your order for an ad, these two steps must be completed - DEADLINE: SEPTEMBER 30TH:

1) Fill out this Senior Yearbook Ad Order Form

2) Make your payment (by card online here, or cash/check)

****Checks should be made payable to CHS with "senior ads" and your student's name in the memo line.

****Check should be in an envelope with "Attention: Athena Davis" on it.

****This can be mailed (CHS, 850 Raider Dr, Cleveland, TN, 37312) -- be sure to allow enough time for it to arrive by the deadline-- or dropped by the school or, if it's not a surprise for your student, your student can bring it to room 220.

****Cash payments also acceptable.

****All payments are due before the ad will be created.
